COVID-19 Vaccination Appointment

Please note, scheduled vaccine appointments are required. No walk-ins.

Vaccine appointments are for Oklahoma residents only. If you are not an Oklahoma resident expect to be turned away.

Welcome to the Oklahoma State Department of Health (OSDH) COVID-19 vaccine registration portal. Here’s how to register:

  1. Fill out a short questionnaire.
  2. If you’re eligible in the current vaccine phase, you’ll be able to schedule your vaccine, pending supply and appointment availability.
  3. If you’re not eligible, you’ll receive an email when your phase is ready for vaccinations.

Where we are now

Current priority groups that are eligible for vaccine appointments as part of Phase 2 of the vaccine distribution plan are:

  • Healthcare workers including expanded healthcare worker groups such as allied health fields and general outpatient health service (i.e. behavioral health, dentists)
  • First responders
  • Oklahomans age 65+
  • Oklahomans under age 65 with comorbidities
  • Teachers and staff in PreK-12 schools and childcare facilities
  • People living and working in congregate locations and worksites
  • Public health staff and state, county, and city government leaders and elected officials
We are actively monitoring the phase progress for our statewide vaccine distribution plan and will update all of our communications accordingly when we have added additional phases/priority groups. Though you are welcome to register via this portal, we recommend waiting until you have confirmation your priority group is being served. This will help ensure you receive both timely and accurate email communication specific to your vaccine appointment.

Please complete the required questionnaire so that we can gather information regarding your readiness for COVID-19 vaccination, and offer guidance and instruction to ensure your safety. You will need to provide the following details:

Demographic details

Such as race, ethnicity, gender, age

Medical history

Current/past ailments and allergy information

COVID-19 history

History of any COVID-19 infections, testing or vaccinations
